SAN FRANCISCO, CA – December 15, 2010 – Health Evolution Partners (“HEP”), announced today that the Health Evolution Partners Growth Fund, along with Dan Kohl and Jack Draughon, launched Halcyon Home Health, a new company being formed to deliver the next generation of home health care solutions based upon clinical excellence and coordinated care delivery.   Halcyon will focus on acquiring and building a network of home health agencies that deliver high quality, cost effective outcomes generated by clinically driven, information based care.
